Terms & Conditions
• In case of a medical emergency during which I cannot be reached, or am incapacitated, I give permission to the healthcare facility chosen by SunRise Church and/or SunRise Preschool (“SunRise”) to provide medical treatment for myself and/or my child(ren). During such medical emergency, I also hereby give permission for an ambulance to be ordered to transport myself and/or my child(ren) to a medical facility should it be deemed necessary by SunRise. I understand my insurance will be used as the primary coverage in the event of a medical emergency.
• I accept personal financial responsibility for any injury sustained by myself and/or my child(ren). I promise to indemnify, defend and hold harmless SunRise (including its staff, leaders and volunteers) for any injury to myself and/or my child(ren) occurring or related to this event hosted by SunRise. I understand all reasonable safety and health precautions will be taken during this event.
• I consent to SunRise having the right to take photographs, videotape or digital recordings (collectively, “Images”) of myself and/or my child(ren) and use such Images in any and all media for the purposes of ministry, promotion and memorabilia. I further consent to the name(s) and identity(ies) of myself and/or my child(ren) being revealed therein or by descriptive text or commentary. I do hereby release to SunRise all rights to exhibit such Images in any form (e.g., print and electronic form) for any such purpose set forth above. I waive any rights, claims, or interest I may have to control the use of such Images. I further understand there will be no financial or other remuneration for use of such Images, either for initial or subsequent transmission or playback.
• I UNDERSTAND SUNRISE (INCLUDING ITS STAFF, LEADERS AND VOLUNTEERS) WILL NOT BE LIABLE FOR ANY DIRECT, PUNITIVE, CONSEQUENTIAL, INCIDENTAL, INDIRECT, EXEMPLARY, SPECIAL, OR ANY OTHER TYPE OF DAMAGES, WHETHER IN CONTRACT OR IN TORT, WHETHER YOU HAS BEEN ADVISED OF THE POSSIBILITY OF SUCH DAMAGES, AND WHETHER (IN ANY PARTICULAR SET OF CIRCUMSTANCES) SUCH DAMAGES ARE REASONABLY FORESEEABLE, IN CONNECTION WITH YOU AND/OR YOUR CHILD(REN)’S ATTENDANCE AND PARTICIPATION IN THIS EVENT HOSTED BY SUNRISE.
